There once was a good man who had to do unspeakable things.
He became sad and alone by acts that could not be told.
His heart that once beat, now cold and incomplete, died and decayed away.
What was left was a shell, a coffin unfilled not worthy for sustainable life.
Living for death. . . Death was his life. . .
Time passed for this man with torment and shame of what he had to relive.
Life was a dream. . .
. . . Dreams are nightmares of things that are best left unseen.
A new heart can grow for this man if given the chance by people willing to pay for his toll. The price is not gold and never grows old, it is a balm for the wounds that can’t heal. . .

I feel I must point out that the expression on his face doesn’t quite seem to fall in line with your description of torment and shame from past regrets.
That being said, I absolutely love the artwork; your ability to render fae-tinged characters is coying and sweet. And your lyrical description is captivating as well, its poetry could almost stand alone.

I would agree with your statement that the face on the syringe is “coy and sweet”, but I believe that its qualities display potential for a gentler side. The face is not the face of the tormented, but the face of hope.
Unlike the piece Angel of Mercy, which has multiple opposing meanings for the syringe symbol. Tinman’s Heart conjures the image of a potential antidote/cure/vaccine.
